Procedure Outline​

What are SIBO and IMO?

SIBO is a bacterial overgrowth, and IMO is an overgrowth of archaea in the small intestine. They produce hydrogen and methane, which cause digestive symptoms such as bloating, gas, loose stools, or constipation.

Diagnostics

SIBO and IMO can be diagnosed using the hydrogen-methane test, which is a simple and noninvasive procedure. It detects the concentration of hydrogen and methane in exhaled breath.

Treatment

Treatment involves the use of antibiotics, prokinetic agents that stimulate intestinal peristalsis and the migrating myoelectric complex, probiotics, a FODMAP diet, and changes in eating habits.

How to Prepare for the SIBO Test?
Don't forget to prepare for the test!

Recommendations

Please be sure to prepare for the test at our center according to our recommendations. These guidelines are in line with the latest scientific literature. Please do not follow the recommendations of other research centers. We perform the test in accordance with the highest standards.

Please read the test preparation procedure carefully. This is very important, as improper preparation may skew the results and require the test to be repeated at a later date.

One month before the exam:

- Do not take antibiotics (furazolidone, nifuroxazide, bismuth preparations, Xifaxan),
- Do not undergo a colonoscopy or fluoroscopy.

One week before the test:

- Do not take probiotics or prebiotics,
- antibacterial herbs (e.g., oregano oil),
- do not take prokinetic medications (Prokit, Tribux),
- do not take high doses of magnesium or vitamin C.

3 days before the test:

- Do not take stomach drops, betaine HCl, or apple cider vinegar,
- Do not take laxatives, such as lactulose,
- Do not take digestive enzymes,
- Do not take supplements containing fructose.

Duration of the diet prior to the test:​

- If you have diarrhea or normal stools, follow the diet 1 day before the test,
- If you have constipation, follow the diet for 2 days before the test (constipation is defined as having a bowel movement 2 times a week or less; if you have a bowel movement every other day, follow the diet for only one day),
- The longer the preparation diet, the lower the bacterial count, and the test may yield a false-negative result. There is no need to follow the diet for any longer.

The day before the test, you may only eat the following foods:​

1) meat, fish, eggs, seafood,
2) black coffee without milk or sugar, black tea without sugar, still water,
3) fats (olive oil, vegetable oil, lactose-free butter),
4) spices—only pepper and salt,
5) rice in small amounts (1–2 tablespoons of cooked basmati or jasmine rice) if you feel weak. Rice contains non-fermentable carbohydrates, but it has a constipating effect, so it’s best not to eat it unless you’re feeling weak,
6) vegans can eat firm tofu.

Do not consume the following products:

1) grain products (bread, rolls, pasta, groats),
2) dairy products (milk, yogurt, cheese, etc.),
3) vegetables, including potatoes,
4) fruits,
5) fruit juices and preserves,
6) nuts and seeds,
7) legumes.

What might a sample meal plan look like 1–2 days before the test:

Breakfast:
Scrambled eggs cooked in lactose-free butter, hard-boiled, soft-boiled, or fried eggs without vegetables or bread, smoked salmon, mackerel.

Lunch:
Meat (chicken, turkey, tenderloin, pork loin, steak) or fish or shrimp (can be grilled, boiled, baked, or sautéed in oil)
without vegetables, pasta, grains, or potatoes.

Dinner:
The same as for breakfast or lunch.

Throughout the day, you can drink plain still water, weak tea without sugar, and coffee without sugar or milk.


12 hours before the test:

- Do not eat anything except plain, still water; you may drink as much water as you like until the end of the day,
- Do not smoke,
- Do not chew gum. ​

On the day of the examination:

- Fast; do not eat anything on the day of the test—this is important,
- You may drink half a glass of still water in the morning; if your test is at noon, you may drink more water in the morning,
- Brush your teeth with xylitol-free toothpaste or just water,
- You may use an antiseptic mouthwash,
- If you wear dentures, do not use denture adhesive,
- Do not engage in any physical exercise,
- Stay seated in one place,
- Do not lie down or sleep (if you are taking the test at home),
- Do not chew gum,
- If possible, do not take any medications; you may take them after the test is complete, unless your doctor has instructed you not to stop taking them.
Medications such as Eutirox, Letrox, and PPIs (omeprazole, Controloc, Emanera, etc.) can be taken immediately after the test is finished,
- You can bring a laptop or a book (the test lasts 3 hours),
- don’t stress about the test—it’s non-invasive,

What are the contraindications for breath tests?

Absolute contraindications include hereditary fructose intolerance and postprandial (or reactive) hypoglycemia.

Is a referral required for the test?

This test does not require a doctor's referral.

Is it hard to drink lactulose?

No, it's a slightly sweet solution that's easy to drink. There are virtually no side effects, except for bloating and gas in cases of SIBO and IMO.

What kind of camera is used to perform the test?

The test is performed using the QuinTron Breathtracker SC device. QuinTron is a device used to conduct scientific research at the world’s leading research centers.

Do we create personalized meal plans for the FODMAP diet?

Yes, if you don't have a personal dietitian, we'll create a FODMAP meal plan based on your preferences.

How much does a SIBO test cost?

The cost of the test is 300 zł

How is a breath test performed?

The patient inhales and exhales into a special bag, and then the staff analyzes the sample using the QuinTron Breathtracker SC device.

Next, the patient consumes 10 g of a lactulose solution (dissolved in 250 milliliters) of water. Lactulose, a synthetic disaccharide composed of galactose and fructose, is not absorbed
in the small intestine and is fermented by intestinal bacteria.

Lactulose moves down the digestive tract, where bacteria break it down into short-chain fatty acids, hydrogen, and carbon dioxide.

The hydrogen dissolves in the blood and is transported to the lungs, where we can measure its presence in exhaled air. Hydrogen can be used by archaea
to produce methane.

The patient repeats the exhalation procedure every 20 minutes for 3 hours, and again at the 90-minute mark of the test. This is important! The 90-minute mark of the test is the most critical in terms of hydrogen levels.

If there is an elevated level of hydrogen and/or methane in the relevant section of the small intestine, this may indicate
small intestinal bacterial overgrowth (SIBO) or an overgrowth of archaea (IMO).
